We require at least 3 days notice for completion of all forms and letters.
Requests should be discussed with reception staff - a consultation may be required.
Most forms will require the doctor to go through patient notes and files before completing the form itself - this can be time consuming.
Any form that requires a persons current health or for their identity to be confirmed will require a consultation with the doctor prior to completion.
All non-social welfare forms and letters are not covered by the medical card scheme.
Charges will apply to the completion of forms and letters.
